I have been asked to notarize a will which includes an affidavit. I personally know the testator and the witnesses.

The affidavit form reads in part "who is personally known to me or who has produced __________ as identification". Since I know the testator, do I write "personally known" in the blank or do I underline "personally known" and write N/A in the blank, or underline "personally known" and leave the blank space a blank space?

You should seek help from the creator of the document, normally an attorney, as wills can be invalidated by the slightest variance.
